You've got your birth plan all written out, the pregnancy has been free of any complications and you are about to experience the birth of your dreams. Suddenly at 38 weeks, your water breaks and there is a complication that necessitates the need to change your plans. This can be a very difficult adjustment if you've never talked about the possibilities of needing to go on to the next best thing. Whether you've planned on a birth at a birth center and ended up at the hospital, planned an unmedicated birth but ended up with an epidural after 36 hours of labor, or planned on a normal, vaginal birth but ended up with a cesarean birth...You made a decision out of love for your baby to go on to the next...best...thing! One of my goals as a doula is to help couples have the best experience possible, regardless of the circumstances. That was the case with this beautiful couple who had to make some adjustments to their birth plan. Even though the birth of their son didn't take place at the birth center, everyone in that room will remember it as one of the most amazing hospital births ever!

An Unavoidable Cesarean

Placenta Previa is a condition where the placenta is either partially or completely covering the opening of the cervix and usually necessitates the baby being born by cesarean. That was the case with this little guy.

Last Friday, I attended my 66th birth as a doula and the first for a scheduled cesarean birth. I've had previous clients go through labor and for various reasons, end up having a cesarean. I have never been able to attend the surgery as they'll only let one support person in. This time, however, I was that support person. This young woman was very brave as she faced what was ahead in the operating room. I was prepared for what I'd see ( since I've watched numerous videos on cesarean birth), but I'd say I was less prepared to witness what women go through emotionally while undergoing a cesarean. I stayed close by her head whispering words of comfort...Letting her know she was doing an amazing job of staying calm even while facing this great fear! "You're SO strong! You're doing beautiful! He's almost here! You did it...There's your son!" And then the moment she first laid eyes on him...

While on the subject of cesareans, if you have experienced a cesarean birth and would like a safe place to share your story and express your feelings, we are holding our first Cesarean Support Group meeting on January 26th. The purpose of these meetings is to provide support, friendship, sharing of birth stories and resources in a nurturing environment. Information will be available that will help you plan for a VBAC, if that is a possibility, as well as find providers in the community that are supportive of VBAC's. We will also discuss ways to help you have a more positive cesarean experience should it be necessary for you to have one. This meeting is not associated with ICAN.

Nothing Quite Like a VBAC!

I know I say that every birth is amazing and they really are...But there's just something extra special about a mama birthing her baby they way God intended after having experienced a previous cesarean! I'll let you peek into what the room looked like shortly after Jessica pushed out her second son Benjamin. That look of peace and determination never left her for a moment through her 12 hours of labor. Every person who stepped into that room was in awe of this woman's strength. I don't think it surprised her husband one bit though...He knew she was determined to do it!

The Birth of Her Dreams!

I love it when a couple does some research on how they can have a better birth experience. This mama wanted a totally natural, unmedicated birth and knew she couldn't do it alone. Her husband was VERY supportive of her decision, but also knew he'd need some help and would benefit by having the assistance of a doula...This is where our lives connect! She makes it eight days past her estimated due date without any cervical checks. Her midwife said, "If we're going to go for a natural birth, no need to check on the cervix!" I LOVED that...It's so true! If you're not in labor, what does it matter what your cervix is or isn't doing?

Water has always been a big part of this family's life and water was where she wanted to labor. So much so that she wouldn't get out when it came time to push. This is how we came about having a beautiful, unplanned water birth at the hospital. The nurses and midwives were in complete awe as they witnessed this strong woman laboring. It had been so long since they had seen an unmedicated birth that they couldn't even remember when...Yes, there were some tears! They were SO compassionate and even allowed Dad to scoop up his baby girl and place her in mama's arms. This is what it looked like...

Need a One Day Childbirth Class?

The Birth Journey workshop will be held January 21st from 10am to 5pm at the offices of Babymoon Inn. This class is designed for couples and is open to anyone regardless of where you'll be having your baby. If you have a due date in February or March, this is the class for you!

I'm thinking that a good time was had by many couples last year for Valentine's Day...November was a busy month for births! I am SO thankful I have the opportunity to support families through one of the most memorable times of their lives...The birth of a child! There were three of them born within five days and I was one busy doula!

Heather's humor was always present throughout her labor and her beautiful smile wasn't far away. Here is that smile as she looked up at her husband with pure joy and the ultimate accomplishment!

Grace endured a very long, difficult labor and didn't complain once. Could it have been because she knew she was surrounded by nothing but love from her husband, mother, mother-in-law, sister, two doulas and a wonderful midwife? Maybe it was the scriptures she had chosen to be read to her as she was laboring. "But you, be strong and do not lose courage, for there is a reward for your work!" ~ 2 Chronicles 15:7. Whatever it was that got her through, she earned the title of "Amazing Grace!" Her little guy had a rough start as well after ingesting some meconium. He needed a little help with breathing and was kept in a higher level nursery under observation for four days. I am happy to report that everyone is home and doing great! Mom and Dad are in awe of this little guy...Isn't he a cutie!

What's in YOUR Doula Bag?

This is the bag that I take with me whenever I leave my house. If you were to open it right now, this is what you'd find: Three client folders (expecting a labor call from any one of them any day!), my camera (to capture those great moments during labor and birth!), peppermint oil (helps with nausea!), a lavender filled squeeze ball, some Cliff bars and an Oriental hand-held fan. Sometimes at a birth I'll use everything in my bag...sometimes I'll only take out my camera. The most important things I take with me aren't actually in my bag. You see, I bring my heart and my hands, my comfort and confidence...I don't EVER want to walk into a birth setting without them!

Currently, the Birth Journey workshop is held once per month on a Saturday. In order to allow more couples to experience this unique childbirth class, I am considering adding a Wednesday/Thursday evening class from 6-9pm each night. If you or someone you know has a due date in November or December, the October class would be perfect. The all day Saturday class will be held on October 22nd and if there's enough interest, we'll add an evening class on October 19th and 20th. The dates for upcoming Saturday workshops are November 19th and December 17th. Classes are held at Babymoon Inn, but are open to anyone regardless of where you are having your baby. Ethan is making sure mommy is comfortable while patiently waiting for his baby brother to be born.

It was way past Ethan's bedtime so he was resting on the couch in the living room of the birth center when his dad ran out and told him it was baby time! He was SO excited as he watched this little body emerge from his mommy. Overcome with emotion, he shouted, "Oh, I love him!" and you can see that love all over his face! What a beautiful way to innocently witness birth; standing beside Daddy while his hands catch your baby brother. Beautiful...just beautiful!

Labor Day 2011!

It was a Labor Day to be celebrated...This little one was in a big hurry to meet her mommy! I arrived at the hospital with Tracy at 2:15pm. She was 1cm dilated and 90% effaced. We walked around for an hour and came back to be checked again...1cm! Jump ahead to 6:42pm when this mama pushes out a beautiful little girl she named Rivers...Happy Labor and Birth Day!

Sushi at a Mexican Restaurant?

You just found out that you're going to have a baby! Now it's time to figure out what kind of birth you'd like. What do you want the environment to be...quiet? conservative? respectful? natural? medical? Do your research and find the provider, facility and support people that will give you the best chances of helping you achieve your desired birth experience. If your desire is to have an unmedicated, intervention-free birth, then choosing to birth in a hospital that has a 99% epidural rate might not be the best environment to achieve those results. You want your body to be free of all electronic equipment? Labor in a nice, warm tub? You'd like to eat and drink whatever you want during labor? You want to walk around outside? Can it happen in a hospital? Yes, it can and I have been privileged to be part of several such births.

If you've had a cesarean birth and are now planning to have a VBAC (vaginal birth after cesarean), going to a care provider that has a policy of only allowing you to gestate to 40 weeks most likely will not get you a VBAC. Do your research and find a care provider that is excited about your desire to have a vaginal birth and will do anything within their ability to help make that happen!

Do you want your care provider to really get to know you and you them? That is going to be a difficult task if you go to an obstetrics office that rotates you through all six OB's in the practice. Not only that, it's hard to get to know someone by spending 10 minutes with them once or twice per month. If that's the kind of care you want and expect during your pregnancy, you probably should check into receiving care from a midwife. A midwife will spend close to an hour with you at each appointment which allows her to get to know you, your body and your baby!

Can you have the outcome you desire even if all the circumstances aren't in your favor? Yes, it is possible...but it would be like craving Sushi and expecting to get exactly what you want at a Mexican restaurant. You better hope they have the right ingredients to make your favorite Sushi. If they do happen to have the ingredients (and they're fresh!), you also better hope the cook who's working that shift knows how to make your Kamakazi Roll and Tobiko! What...no Edamame?

Epidural Anesthesia

This is a great video to watch if you have no idea what is involved in getting an epidural. Where you see the anesthesiologist moving the needle in different directions, he's trying to find the exact space in your spine to place the catheter for the epidural. Since no two spines are alike, he has to go by feel.

There are a few things that are left out from the video that are important for you to be aware of. You will also need to be catheterized to empty your bladder since you can't get up to go to the bathroom. You will have an oxymeter placed on your finger to tell them what your oxygen level is. Often times, your blood pressure will drop too low so you'll be given something to bring it back up. You may itch, shake or run a fever. An epidural will often slow a woman's labor and she is three times more likely to be given pitocin to speed things up. All of these are normal responses and are monitored closely by the medical staff.

The pushing stage of labor is particularly slowed due to the relaxing of the pelvic floor muscles. These muscles are very important in this stage of labor as they're used to guide the baby's head so that it enters the birth canal in the most optimal position. This can lead to an increase in the use of forceps or vacuum extraction to help get your baby out.

Don't get me wrong...epidurals have their time and place. Many women have been saved from a cesarean section by receiving an epidural. The following are just a couple instances of when an epidural is beneficial. If a woman has been in labor for an extended period of time with no sleep, her body can become extremely exhausted and begin working against her. If a woman is struggling with high blood pressure during labor, often times she may benefit from an epidural due to its affect on blood pressure.

As a doula, I support a woman's decision of whether or not she wants an epidural. If I can help her have a labor and birth experience that allows her to get through without one...That's icing on the cake!

We made it to Disneyland!

I tell parents that labor and birth is like going on a trip to Disneyland. You could have a flat tire and you might come upon some road construction; there may even be a detour...BUT you're still going to Disneyland. That was the case with this mama whose water broke and no contractions in sight for at least 30 hours. The Cook Cervical Balloon worked great to get her to 4cm and then she worked so, so hard over the next 18 hours to get to 6cm. Things didn't quite go as planned, BUT they still made it to Disneyland and when the ride was over, this gorgeous little guy awaited their snuggles!

I love that there are several options for birth centers in the Phoenix area and I've had the opportunity to tour them all. The first one I visited was http://babymooninn.com/ where I met Julia the Director, and Certified Nurse Midwife Connie Garcia. This is a quaint little place that has the cozy feeling of a bed and breakfast. There's a nice, large birthing tub in the same room as the full size bed and a gorgeous spa shower just steps away. As part of their package for first time mamas, they include doula services and a childbirth class!

Last Saturday, I went to the open house of the Women's Birth and Wellness Center in Mesa http://azmidwifeonline.org/. This is a non-profit birth center and at the heart of it is Family Nurse Practitioner, Claudine Calligan. Claudine is also a Certified Nurse Midwife and works alongside her two daughters; one who is becoming a Nurse Practitioner and the other is an RN. This is quite the family operation and quite the ministry as well. If you've had a cesarean and desire to have a VBAC outside of a hospital, stop in and see Claudine. She will do an interview with you to see if you are a candidate for birthing at the birth center.

The most recent birth center I visited was http://blossombirthcenter.com/ in central Phoenix. I had the pleasure of meeting with midwife Mary Langlois who shared the story of how Blossom Birth Center came into existence. With having Dr. Nichelle Whitehead as part of the team, they have the added benefit of having an experienced OB who has a heart for natural birth! They did a wonderful job of designing the two birthing rooms with tubs in each room and a common kitchen area that is state of the art. This is a full service wellness center that includes accupuncture, aromatherapy, massage therapy, and even a one-stop lactation center!

We are truly blessed to have so many options of birth centers and wonderful midwives. I am excited to meet another great midwife, Stephanie Soderblom. I've heard nothing but wonderful things about her and the work she does through http://www.nurturingheartsbirthservices.com/

If only I could have one more baby and have my dream birth...On second thought, I'll just enjoy my grandbabies and help other mamas have their dream birth :0)

Merry Christmas from my family to yours!

It has been a whirlwind of activity since my last entry in July! As of December 1st, I am now an Arizonan once again...and VERY happy to be here! I miss my doula friends, midwives and family (past clients too!) so it is very bittersweet.

I'm looking forward to building new relationships with the local midwives and attended my first Birth Circle meeting last night. It felt so good to be in a group of women who share the same passion of birth and babies.

Who will my first Arizona mommy be that I will have the privilege of supporting through the amazing experience of labor and birth? We made the 35 minute drive to the birth center and were told that it was still early labor so we drove back home to labor a while longer. I had suspected that baby was in a posterior position so as soon as we got back home I had Emily get into a couple different positions to encourage baby to turn. After about an hour, you could tell he was no longer posterior and labor quickly progressed. We had been home a couple of hours when we knew it was time to head back to the birth center. About 15 minutes after we left the house (remember...it's a 35 minute drive!) little Landon tried to be born in the car on I-5! I told Dad to go ahead and pull over...we were not going to make it to the hospital without a police escort. Right where we stopped, a policeman had pulled over a car on the other side of the freeway so we flagged him down. In just a few short minutes, an ambulance was zooming off to the hospital carrying a mama who was determined not to have her baby born in the ambulance. She got her wish...4 minutes after pulling into the hospital parking lot, little Landon made his arrival!

"I felt like it was more uncomfortable than painful... This is weird but I've broken bones, had a root canal, stubbed my toe and on a pain scale those were all much worse than labor... It felt like there was pressure with no where to go but out somewhere either through moaning or humming or whatever... I run out of words when trying to describe the joy in the room when a woman pushes her baby out after being told by her OB, "I'm sorry, but you'll have to have another c-section!" This dad was overcome with absolute love and adoration as he encouraged his wife during the final stages of pushing. As he saw his son's head beginning to crown...Well, those moments are unspeakable so I'll leave you with this!

Is Your Baby Too Big?

How many times have you heard a woman say, "My OB says my baby is too big to fit through my pelvis so I have to have a c-section or else be induced before my due date?" Maybe you've been told this. Well, a big baby doesn't have all the extra weight in their head only...It's dispersed throughout their body in squishy parts like buns and thighs. A larger baby has less room to become malpositioned or have a compound presentation since pelvic room often dictates a ONE-WAY fit!

Certain positions will make more room for your baby as well. Did you know that squatting can open your pelvis by as much as 30%? Squatting is a great position to use toward the end of pushing to make room as the baby's head is descending lower into your vagina and is just visible.

Tips for giving birth to a big baby:

Allow labor to begin on its own without being induced.
Use a variety of positions during labor and pushing.
Consider a squatting position if needed.
Ask for as much time in labor as possible, especially in pushing.
Use additional labor support such as a doula, to help you with pain relief techniques and progress.
Consider the benefits of a natural birth since pushing is more effective and progress is better than if you use pain medication. Also, pain meds can make it harder to get into certain positions such as squatting.
Get regular chiropractic adjustments so that your spine and pelvis are in alignment before you go into labor.
Practice your positioning and exercises during pregnancy so that your body is toned and has the stamina to stay in these positions, if needed, for 30 minutes or longer during labor.

Kelly was like a kid anticipating a trip to the candy store at the thought of being in labor! After 23 hours of tough labor and sheer determination, she had an awesome VBAC! A BIG thanks to Dr. Christina Kelly (Tacoma) who was one of Kelly's biggest cheerleaders! She comes highly recommended if you have had a prior c-section and desire to have a VBAC!

Young, single, and unexpectedly pregnant...Not ready to be a parent. Faced with the choice of terminating a life or giving life, this young woman made the decision to carry this baby for another couple to parent. During the pregnancy, she went to great measures to make sure she was doing what was best to grow a healthy baby...Eating healthy, exercising, and no drugs or alcohol. She even went so far as deciding to have an unmedicated birth! Considering the fact that 76% of women today have an epidural during childbirth, this was AMAZING! I was honored to be the doula for this courageous, young woman who gave birth to a 9lb 2oz baby boy after only 3 1/5 hours of labor. The room couldn't have been any more peaceful as this beautiful little boy was born into the water and placed on his birth mama's chest. He was showered with kisses and lovingly admired before being placed into the arms of the wonderful couple that would be his parents. Little Brody...You're a lucky little guy and loved by so many!

Dr. Stuart Fischbein shares his thoughts on delayed cord clamping. There is more information on this in an article in TIME magazine. A study was done on a group of newborns where 50% of the infants (21 out of 41) who had the umbilical cord clamped/cut before they took their second breath showed moderate to severe signs of respiratory distress. Of the remaining infants whose cords had been clamped and cut sometime after the second breath was taken, only six suffered the same symptoms! From 5cm to birth in 24 minutes!

There's nothing like being comforted by the loving arms of your Mama!

It took 15 hours of slow, tolerable labor (14 of those at home) to get to 5cm and then Elijah Mekhai was born 24 minutes later! Monet requested an epidural 45 minutes after we arrived at the hospital but agreed to try the tub first. She was so relaxed in the warm water that she thought the nurse had given her a shot of Fentanyl. No medication was given...just the natural releasing of endorphins! Next time girl, you better just plan on having your baby at home!

Meet Elijah Mekhai who was born on January 4th, 2010 at 1:24 pm. He weighed 6lb 12oz and is 19 inches long.

One hour old and already breastfeeding like a pro! You've done an awesome job and I have no doubt that you're going to be a great young mom.

2009 was the year I made the decision to follow my dreams of becoming a doula and helping women have the best birth experience possible. I had already attended 24 births so this was not a new passion. It began in June when I went through the Doula training and started the certification process. Since then, I've had the privilege of being a part of 10 wonderful births as an official doula...each one unique and a miracle!

Am I REALLY in labor?

Amber had her checkup with her Midwife at 4:30 pm and to the surprise of everyone, she was already dilated to 8cm without realizing she was in active labor! I got the call at 5 pm and headed to their home as quickly as I could in rush hour traffic. Amber was able to relax in the warm water of the birth tub for about 45 minutes before Baby Nathan gently slipped out into the water and into the hands of his Daddy.

Congratulations to Amber, Jun and big brother Caleb on the birth of little Nathan! Born at home in the water on November 24th, 2009 at 7:15 pm. He weighed in at 7lb 14oz and is 20 inches long.

After a very intense 48 hours of labor, Dahlia Lorraine was born by cesarean section at Swedish Ballard on November 21st, 2009 at 6:46pm. She weighed 7lb 14oz, was 19.75 inches long and has adorable red hair...Hmmm, she was a little stubborn by trying to come out sunny-side up! What is a Doula?

The word "doula" is Greek in origin, and has come to mean "woman helping woman" in childbirth. A doula is a non-medical labor support assistant. She provides a birthing woman with physical, emotional and informational support at childbirth, whether the birth takes place in a hospital, home, or childbirth center. Her primary concern is the mother, although she may also support other members of the family who are present. Doctors, nurses and midwives usually are not in a position to provide this continual support. As medical professionals who may be caring for more than one patient at a time, they cannot stay in the room with the mother. They also need to concentrate on the more clinical aspects of childbirth.

A doula's presence does not make a birth partner's presence unnecessary. To the contrary, a doula's presence frees the partner from certain aspects of concern and allows the partner to interact more closely with the birthing woman. A doula's purpose is to aid the mother in labor and help her have the birth experience she wants, thereby leaving her with a sense of empowerment and a beautiful, positive birth memory.

The Role of a Doula:

Most doula and client relationships begin a few months before the baby is due. During this time, they establish a relationship that gives the mother complete freedom to ask questions, express fears and concerns, and take an active role in creating a birth plan. Most doulas make themselves available to the mother by phone to answer questions or explain any developments that may arise in pregnancy. Doulas do not provide any type of medical care. However, they are knowledgeable in the medical aspect of labor and birth so they can help their clients get a better understanding of procedures and complications that may arise in late pregnancy or during the birth process. During labor, doulas are in constant, close proximity to the mother at all times. They can provide comfort with pain relief techniques, such as breathing, relaxing, massage and laboring positions. Doulas also encourage participation from the partner and offer reassurance. A doula acts as a silent advocate for the mother, encouraging her in her desires for her birth. The goal of a doula is to help the mother have a positive and safe birth experience, whether the mother wants an un-medicated birth or is having a planned cesarean birth. After the birth, many birth doulas will spend a short time helping mothers begin the breastfeeding process and enoucourage bonding between the new baby and family members.

Today, many fathers are taking a more active role in the birth process, but some partners feel that this is a huge expectation, and would rather be able to enjoy the birth without having to stand in as labor coach. With a doula as a part of the birth team, a father can do whatever he feels comfortable with at each moment. Doulas can encourage the father to use comfort measures and can step in when he needs a break. Having a doula allows the father to be able to support his partner emotionally during labor and birth and also enjoy it himself without the pressure to remember everything he learned in childbirth class!
